How does anyone handle updates that they aren't going to push out to the environment? Do you decline the updates? Is there any benefit to doing this task?

Interested in how others do this in their org.

We decline the updates. The benefit being that those updates will not show up as needed in your reports. Makes things look a little cleaner and easier when going through which updates your systems needs. And you can always approve them later if you find you want to approve them at a future time.

Which brings me to another question... Am I alone in the struggle with reporting?

I have been tasked with trying to build a spreadsheet of all patches released every month. A system owner will then have to evaluate whether they would like the patch turned on or not. If the patch is not wanted, it'll be declined and moved to a declined state in the spreadsheet.

So first thing I did was pull a report of needed patches from WSUS. When I pulled this tho, it didn't give me the info I wanted. There were patches that were approved because there were machines out there that were requesting the patch it showed up on the report. I don't want to have to look up every KB in WSUS and see if that patch is actually approved or not. I can't seem to figure out a way to get through the first pass... I anticipate that once I get WSUS cleaned up (declining unwanted patches, etc.), it will be less of an issue in the future. But I am overwhelmed with how I will get there...

Does anyone have any thoughts on this? Maybe words of encouragement? :P

I don't know about words of encouragement, but I can certainly relate.  When I was one of the people responsible for patching where I work, one of my big complaints was how Microsoft would roll stuff up into other patches.  If I encountered a patch that negatively impacted an application or system, we'd decide to hold off on deploying until there was a resolution -- only to encounter the same problem again when that particular patch was rolled up inside another in a future patch release.
